Quilters Favorite Photo Quilt Competition II

The Contest: From May 1st, 2010 through October 31st, 2010.  Mosaic Quilt Studio will promote, by means of Internet promotions and various quilt shows the opportunity to submit entries to win
a grand prize as detailed below. PRINT / RADIO / TV / WEBSITE advertising may also be used to promote the contest beginning May 1st, 2010 through October 31st, 2010.

Eligibility: In order to submit entries, entrant must be at least 13 years of age on May 1st, 2010. Each entrant is allowed to enter one (1) quilt. Only one entrant per household. Any
individuals (including but not limited to employees, consultants, independent contractors, and interns) who have, within the past six months, performed services for Mosaic Quilt Studio or any
organizations responsible for sponsoring, fulfilling, administering, advertising or promoting the contest or supplying the prize, and/or their respective parent, subsidiary, affiliated and successor
companies, and immediate family and household members of such individuals, are not eligible to enter or play. “Immediate family members” shall mean parents, stepparents, children,
stepchildren, siblings, stepsiblings, or spouses. “Household members” shall mean people who share the same residence at least three months in one calendar year.

How to Enter: (I) Upload your Quilt images following the video instructions then complete and submit the official online entry form  on or before October 15th, 2010, or (ii) minors 13 to 17 years old must mail the printable Entry Form to Mosaic Quilt Studio. You can find the requested information at: www.TheQuiltPattern.com Entries must be postmarked by October 15th, 2010 and mailed to Mosaic Quilt Studio, Quilters Favorite Photo Quilt Contest, c/o Mosaic Quilt Studio, 917 Fremont Ave PMB138, South Pasadena, CA 91030.

The photo you upload must not be a file that is larger than 30kb. (files that are very large overload the server) Please check your file before loading on the site. If the photo is larger then 30kb, it will be removed from the website and disqualified.

All contest entries become the
property of Mosaic Quilt Studio and will not be returned and (name, city, quilt images and comments) may be used by Mosaic quilt Studio for any purpose and in any media in perpetuity.

How to Win: The Grand Prize winner will be the quilt entry with the most qualified votes on or about October 31st, 2010. In the event that the original Grand Prize winner does not meet Official
Rules or eligibility requirements, or he/she or parent or legal guardian forfeits the Grand Prize, Mosaic Quilt Studio will award prize to first alternate. Process will continue until Grand Prize is
awarded. All entries must be received on or before October 15th, 2010. The selection will be held the week of October 30th, 2010. Decision of the judges and Mosaic Quilt Studio is final. The grand
Prize winner will be notified by email, mail or phone and must meet all eligibility requirements.

By entering, each contestant agrees to be bound by the rules. Mosaic Quilt Studio, its promotional agents and contest co-sponsors are not responsible for illegible, lost, delayed or
misdirected email or graphic files, errors in faxes or shipments, busy phone signals, or incomplete entries.

Grand Prize: (One) Grand Prize winner(s) will each receive one Music themed quilt top (Wall hanging) that was made and signed by Tammie Bowser, the creator of Quilted
Photography. Approximate total value of the grand prize is $450. The quilts shown are examples only and are not the actual Grand Prize Quilts. Consolation Prize: All qualifiers
will receive a Mosaic Quilt Studio Discount Card worth $10 for each entry submitted (Maximum $30 for 3 entries).

The Grand Prize is non-transferable. There may be no cash or other substitutions of prizes by winner. However, Mosaic Quilt Studio reserves the right to substitute similar prizes of equal or
greater value in the event the above prize is unavailable for any reason. Individual, parent or legal guardian of winner is solely responsible for all applicable federal, state and local taxes with respect to entire prize.

Winner or Parent or legal guardian of Grand Prize winner must sign an Affidavit of Eligibility, Release and Indemnification Form (“the Form”) that : a) releases, indemnifyies and holds
harmless Mosaic Quilt Studio its parent, affiliated, subsidiary and successor companies, and its respective officers, directors, employees, agents, and representatives, advertising agencies and
promotional sponsors (collectively the “Release”) from and against any and all harm, expense, liability or injury that may be sustained relating to or arising from participation in the contest or
acceptance or use of the prize; and b) authorizes the Releasee to use the winners’ names, likenesses, photographs, voices and biographical data for any purpose, in any media, unless prohibited by law.

Prize will not be released until Mosaic Quilt Studio receives all required documents. Failure of Grand Prize winner to return the Forms within the required timeframe will result in automatic
forfeiture of any prizes and all such prizes shall be awarded to an alternate winner. In the event of a prize forfeiture, the winner or parent or legal guardian of the winner forfeiting prize will sign a
Prize Forfeit Form and return to Mosaic Quilt Studio within ten (10) days of receiving the Form.

Unforeseen Events: Mosaic Quilt Studio reserves the right to change this contest at its sole discretion at any time that Mosaic Quilt Studio become aware of any circumstances or
occurrence which would materially affect the contest security or fairness. Mosaic Quilt Studio reserves the right to announce any contest rules changes over the air, in print or posters. Mosaic
Quilt Studio is not responsible for the inability of the winner to accept the prize for any reason.

List of Winners: For a copy of winner list, send a self-addressed, stamped envelope to: Mosaic
Quilt Studio, 917 Fremont Ave. PMB138, South Pasadena, CA 91030. In your request, state that you would like a copy of the “winners list” between May 1st, 2010, and December 31st, 2010.
