Quilters Favorite Photo Quilt Competition II Online Entry Form

Submiting an entry form is required to receive anyprize .

Participants must be 13 years of age or older Mosaic Quilt Studio employees and their relatives are not eligible to register to win.

No purchase necessary. Odds of winning depend on total number of entries received and the number of qualified votes. One qualifier per household.

Prize package includes:

Grand Prize: (One) Grand Prize winner(s) will each receive one Music themed quilt top (Wall hanging) that was made and signed by Tammie Bowser, the creator of Quilted
Photography. Approximate total value of the grand prize is $450. The quilts shown are examples only, and are not the actual Grand Prize Quilts. Consolation Prize: All qualifiers
will receive a Mosaic Quilt Studio Discount Card worth $10 for each entry submitted (Maximum $30 for 3 entries).

The Grand Prize is non-transferable. There may be no cash or other substitutions of prizes by winner. However, Mosaic Quilt Studio reserves the right to substitute similar prizes of equal or
greater value in the event the above prize is unavailable for any reason. Individual, parent or legal guardian of

winner is solely responsible for all applicable federal, state and local taxes with respect to entire prize.

Winner or Parent or legal guardian of Grand Prize winner must sign an Affidavit of Eligibility, Release and Indemnification Form (“the Form”) that : a) releases, indemnifyies and holds
harmless Mosaic Quilt Studio its parent, affiliated, subsidiary and successor companies, and its respective officers, directors, employees, agents, and representatives, advertising agencies
and promotional sponsors (collectively the “Release”) from and against any and all harm, expense, liability or injury that may be sustained relating to or arising from participation in the
contest or acceptance or use of the prize; and b) authorizes the Releasee to use the winners’ names, likenesses, photographs, voices and biographical data for any purpose, in any media, unless prohibited by law.

Prize will not be released until Mosaic Quilt Studio receives all required documents. Failure of Grand Prize winner to return the Forms within the required timeframe will result in automatic
forfeiture of any prizes and all such prizes shall be awarded to an alternate winner. In the event of a prize forfeiture, the winner or parent or legal guardian of the winner forfeiting prize will sign
a Prize Forfeit Form and return to Mosaic Quilt Studio within ten (10) days of receiving the Form.

Consolation Prize: All qualifiers will receive a Mosaic Quilt Studio Discount Card worth $10 for each entry

submitted (Maximum $30 for 3 entries).

By participating, all entrants acknowledge and agree that they have entered the contest of their own free will, that the full rules and details of the contest have been made available to them in
writing and they therefore understand and agree that neither Mosaic Quilt Studio their agents, affiliates, sponsors, representatives or employees have any liability with respect to the
contestants’ participation in the contest or winning a prize and are not liable for any damages out of acceptance and use of a prize. By entering this promotion, participants agree to be bound by these rules.
